1. The student or the student's legal guardian allows FDS to maintain and transmit the student's course recordsand other information on this form as prescribed by the MTO

2. This information shall be kept in strict confdence and shall be transmitted in accordance with MTo directivesfor the sole purpose and only as it pertains to the MTO regulations for approved beginner driver education

3. The student will be certifed online electronically upon course completion with a minimum passing grade of70% on all in-class , online classes and in-car assessments As mandated by the MTO, Ontario driving schoolsare not able to issue drivers licence certifcates. instead,students must request a drivers licence history (DLH)

from Service Ontario ($12)

4. *MTO regulations stipulate that the course must be completed within one year of the course start date, if astudent has not completed within the l year they will have to return to class and a fee will apply. We don'thonor any services after 360 days included in car and in class services

5. Fanshawe Driving School requires the student to be ready at the agreed upon time for in-car appointmentsStudents must have Gl present for every session.

6. NO REFUND CREDIT ONLY

7. Individual lessons (private lesson) must be paid in full prior to the in-car session

8. Fanshawe Driving School reserves the right to cancel the in-car appointments on short notice due to theweather, or other circumstances beyond their control.

9, lf an unforeseen circumstance arises, such as a death in the family and a student misses a lesson, the studentwill not be charged. lf there are no extenuating circumstances, in-car appointment cancellations made withless than 24 hours notice are subiect to a charge of the hourly rate of $45.00

10. Fanshawe Driving School will charge $35.00 for dishonored cheques and balance must be paid in full before

the student begins their in-car lessons.

11. Fanshawe Driving Course fees , in car lessons and deposits are non-refundable . You can transfer to a friendor instore credit. lf payment terms that are not met, services will be suspended until terms are fulflled.

12. Students are responsible for all outstanding balances and must read all terms and conditions before signing

(required by MTO).

13. You will be subscribed to Fanshawe Driving School newsletters to stay updated with latest offer and driving

tips and tricks.

14. Price match is only valid for the packages exact match with Fanshawe Packages description

15.$55 fee is applicable for changing classes from online to physical classrooms.

16. 0ur gift cards are valid for one year from the date of purchase, giving the recipient plenty of time to redeemthem for the perfect item. t's important to note that any gift cards that have expired will no longer be valid andcannot be used towards a purchase or refunded, We encourage our customers to make the most of their giftcards before they expire to ensure they get the full value of their purchase
